Students interested in participating in extramural sports during the 2009-2010 school year will need to download the application from the SDHC Athletic Department.
- The completed ADA-1 (middle)
- Two (2) notorized medical release cards
- Printout of insurance card (select Purchase Online from the website link)
- Physical Form (EL2) signed by doctor
Completed packets must be returned to Coach Hitchcock before participating in try-outs or practice. Athletic packets will NO LONGER be available in the Student Affairs Office. If you have any questions please call 975-7665 vm 486 or email Coach Hitchcock.
