How do you enroll your child?


DOCUMENTS REQUIRED FOR REGISTRATION :
*Proof of Address in Parent/Legal Guardian's Name.
Documentation that may be used to verify an address are listed below:

a. Recent Electric or Water Bill (No Telephone or Cable Bills)
b. Copy of Lease Agreement or Mortgage Contract
c. Current Year Tax Receipt Showing Homestead Exemption
d. Affidavit ( if living w/ second party; administrative approval required)to Show Address (after       Administrative approval)
* Proof of Special Assignment (if not residing in Adams School district)
* Immunization Records and Current Physical (within the past year)
*Birth Certificate
* Report Card and/ or Withdrawal Form from Previous School
